In order to better serve our community Southwest Community Health Center, Inc. is open 6 days a week from 8:30am - 4:30pm. Patients wishing to schedule an appointment should call 203.330.6000. We also offer expanded hours of service during the evenings and weekends to help those who may not be available during our regular hours of operation. We encourage you to schedule an appointment for your visit. Having a scheduled appointment will reduce your wait time. We welcome walk-in visits for patients who are sick and need to be seen immediately. Some departments may offer appointments during the hours of 12:00 noon - 1:00 pm based on staff/provider availability.
In order to facilitate your registration, we ask that you provide your insurance information at the time you schedule the appointment. Should you find it necessary to cancel your appointment please give us a 24 hours notice. This will allow us to give the appointment slot to another patient.
On the day of your appointment we will require that you show us identification, proof of address, and insurance information. If you are uninsured you can apply for our Discounted Fee Schedule Program by completing an application and providing proof of available income. Patients should always plan to arrive at least 15 minutes before their scheduled appointment.
