Welcome to our client information space. This space is reserved for all information related to client/patient services and our patient portal. If you scroll to the right, you will find links to our client forms page and our client portal page. In our client forms area, you will be able to download all patient registration and consent forms  if you wish to complete these documents in the comfort of your own home. In our client portal section you will have access to your patient portal which holds all of your Southwest CHC medical records, treatment plans and any other information pertinent to your health and wellness. Please feel free to browse around and become familiar with the client information section of our website as we will be enhancing this page in the coming months to include health topic discussions, healthy eating tips and much more information.

All patients and visitors on official business with Southwest CHC have the right to receive information in a language they understand, free of charge. Southwest complies with the Department of Health and Human Services' Section 1557 Rule of the Affordable Care Act which sets guidelines about language assistance for people with limited English proficiency (LEP), or those who are deaf or hard-of-hearing, and takes reasonable steps to provide meaningful access to people with limited English proficiency who might require assistance within our sites.
