Southwest CHC offers recipients the choice of the Moderna, Pfizer or Johnson & Johnson vaccine. We are currently offering the COVID-19 vaccine for individuals over the age of 12 years old. *proof of age will be required at the clinic
You can expedite your experience by scheduling an appointment but please know that this is NOT a requirement. If you do not have a VAMS account, our staff can create one for you. Federal Identification is preferred but not required.
Scheduling a Vaccine Appointment
You can schedule your COVID-19 vaccine through the Centers for Disease Control and Prevention’s (CDC’s) Vaccine Administration Management System (VAMS) or by calling 877-918-2224 or 211 or walk in without an appointment. Please do not call Southwest CHC to book an appointment.
If you schedule through 211, you will schedule your second dose appointment at the same time you schedule your first dose. If you schedule through VAMS, you should return to VAMS to schedule your second dose.
Please note that every individual MUST have their own unique email to be vaccinated with the Vaccine Administration Management System (VAMS). For example, if you are registering your elderly parent and you use your email, you will not be able to be vaccinated using that same email. Those needing assistance will need to contact a family member or friend to assist you. Using VAMS is a Federal requirement at this time.
Please fill all required fields attesting that you are currently eligible and provide the requested information. This will enable State of CT staff to upload your information into VAMS. Within 24 hours each individual will then receive an email from the CDC/VAMS telling them to now register in VAMS and select a vaccination appointment. Make sure to check your email Spam/Junk folders for the email.
