McKinney Homeless Program

Homeless patients and families can access all services at SWCHC, with additional interventions available to meet the specialized needs of this population. A team of clinicians and support staff visit 6 licensed shelter clinics on a rotating weekly schedule, with referrals for specialty care, and intensive medical follow-up performed at the sites as needed. Through the use of our mobile medical bus, our provider team is able to offer care throughout the community and non-traditional spaces.
