Awards & Recognition
Each year, the Health Resources and Services Administration recognizes health centers nationwide for measurable achievements in clinical quality, health outcomes, access to care and health information technology. Based on 2025 Uniform Data System reporting, Southwest Community Health Center earned six 2026 Community Health Quality Recognition badges—reflecting both the quality of care we provide and our continued commitment to improving the health of the communities we serve. 2026
Preventative Health
Recognizes achievement across a broad range of preventive health measures, including screenings, early childhood assessments, tobacco-use intervention and nutrition counseling. Recipients met or exceeded at least five of HRSA’s ten preventive health targets.
High-Value Care
Recognizes health centers that demonstrated significant improvement in clinical quality while keeping the growth of total costs per patient within HRSA’s established benchmark.
Advancing HIT for Quality
Recognizes the effective use of health information technology to improve care, including telehealth, online clinical information exchange, patient engagement and the collection of data related to patients’ health needs.
Improving Health
Care Access
Recognizes health centers that expanded the number of patients served—including patients receiving behavioral health, dental, vision or patient-support services—while demonstrating strong or improving clinical quality.
Health Center Quality Leader—Bronze
Recognizes health centers whose overall clinical quality performance placed them in the top 21–30% of eligible Health Center Program organizations nationwide.
National Quality Leader—Heart Health
Recognizes health centers that met or exceeded national targets for at least three of four heart-health measures: blood-pressure control, tobacco screening and cessation intervention, statin therapy, and appropriate aspirin or antiplatelet use.

The American Heart Association's Check. Change. Control. CholesterolTM Gold Award 2025.
Southwest Community Health Center, Inc. received the American Heart Association (AHA) Gold Level recognition as part of the 2025 Check. Change. Control. Cholesterol™ Achievement Awards. Data submitted demonstrated an exceptional ≥70% appropriate statin therapy use across the at-risk patient population.
