Services

Southwest Community Health Center is a non-profit organization dedicated to providing high quality care that includes medical, dental, and behavioral health services to individuals and families in the Greater Bridgeport area. In additional, Southwest also provides health education, disease prevention programs, community outreach, and enrollment services. We have a variety of finance options available to assure that we accommodate individuals who may not be insured. We strongly believe our clients have the right to a quality of care that meets their own needs regardless of their ability to pay.  Our health care facility maintains a friendly, professional and efficient health care delivery. Located in different areas around Bridgeport, our sites provide different services including:


46 Albion Street

Our newest site in Bridgeport offers a full service that includes internal medicine where a patient can receive urgent care and same day appointments, women’s health, dental, behavioral health, pediatrics, and HIV/AIDS testing and treatment.

510 Clinton Avenue

Our Clinton Avenue site specializes in women’s health. Complete gynecological care includes prenatal care, pregnancy testing, management for high-risk pregnancy, and family planning. In addition, patients can receive pap smears, cancer screening, mammography, ultrasounds, and STD prevention and screenings.

1046 Fairfield Avenue

This site concentrates on behavioral health treatment including psychiatric evaluation, treatment, and therapy for children and adults. Clinicians provide individual, family, and group counseling. Patients experiencing substance abuse can participate in the Intensive Substance Abuse and Mental Health program and seek continuing care as well.

Our Marina facility offers internal medicine and pediatric services, in addition to a Homeless Health Care Program. Homeless patients and families can access all services provided by Southwest with additional programs available to meet the needs of the client. Our clinicians visit license shelter clinics with referrals for specialty care and intensive medical follow-up appointments when necessary.

1020 Fairfield Avenue

This site caters to women, infant and children and is known as our WIC program. It provides nutrition education, health care referrals, and breastfeeding promotion and support. A SNAP Outreach worker is accessible and assists clients with SNAP (Supplemental Nutrition Assistance Program)/Food Stamps enrollment.
